Dogworks aim to provide a professional, caring service to all our patients. Dogworks requires you to read the terms of business below and please complete this Veterinary consent/Referral form 

All small animal patients treated by Dogworks need a consent/referral form signed and dated by your vet before assessment and treatment can begin. Once you have the referral signed please contact us to arrange an appointment.

Payment of the session must be made at the end of each treatment.

There will be times when you need to cancel or re-arrange an appointment we require a 48 hour notice period. If cancellation or you do not attend your session within 48 hours of your appointment, the session will be charged at full price.

If your dog has any contagious disease or illness we will need to re-arrange the appointment and the cancellation policy applies.

For health and safety if you have any other dogs in your household please only bring the dog referred for treatment to your session. Please ensure they have toileted before the treatment session.

By signing the Consent form and attending the appointment you are agreeing to the additional terms and conditions set out here.

You agree to be Covid Safe abiding by the changes that you will be made aware of by email or message. You agree to abide by the Covid safe protocols to ensure the continued safety of staff and owners.  If you have any symptoms of Corona Virus these include a high temperature, a new cough or have lost your taste or smell please cancel your appointment at no charge and re-arrange another appointment once fully recovered.

General Data Protection Regulation (GDPR) no personal information will be shared with any third parties. Only at the request of the owner will any information be passed on to veterinary professionals. Information is kept in locked cabinet and destroyed after 5 years as in line with legislation.

Electronic messages are encrypted end to end and they will only be used if the owners are happy to communicate this way.
